Community Care

The Dimock Center’s Community Care Department provides comprehensive, trauma‑informed services for individuals living with HIV and community members seeking STI prevention, counseling, and testing. We take a holistic approach that integrates medical care, mental health services, case management, psychosocial support, and prevention education.

Who We Serve

  • HIV services: Individuals who are HIV positive
  • All other services are open to all
We accept referrals from within Dimock Health Center clinics and outside the health center.

What We Offer

HIV Services (for HIV‑positive individuals)

  • HIV testing and counseling
  • Nurse medical management
  • Medication education and adherence support
  • Viral load monitoring and overall health support
  • Individual psychotherapy
  • HIV support groups
  • Mental health services
  • Crisis intervention
  • Psychiatric referrals and care coordination

STI Prevention, Counseling & Testing (open to all)

  • Routine and risk‑based STI testing
  • Prevention counseling
  • Safer‑sex and harm‑reduction education
  • PrEP and sexual health counseling

Psychosocial Support Services

  • Individual and group‑based psychosocial support
  • Coping skills development
  • Support around stigma, disclosure, and relationships
  • Stress management and emotional support

Medical Case Management

  • Assignment to a dedicated medical case manager and/or psychosocial advocate
  • Assistance navigating the healthcare system
  • Appointment scheduling, follow‑up, and continuity of care

Care Navigation & Referrals

  • Help accessing housing, legal services, food assistance, and other resources
  • Close coordination with on‑site and off‑site providers
