Community Care

We care for our Community.

Community Care

Our Community Care team continues to provide intensive support services and medical care to people living with HIV/AIDS.  We work to reach out into the community to increase our care to the people who need it, and provide help for them to continue the best course of care for themselves and enable their self-sufficiency.

The Dimock Center’s Medical Case Managers (MCMs) and Psychosocial Advocates help clients to navigate through our multidisciplinary clinic, as well as to outside facilities. Our Nurse Medical Managers provide medical care and are in close contact with the MCMs and Psychosocial Advocates. All clients also have access to support groups, educational meetings and referrals. The comprehensive care we deliver seeks to help our Community Care patients keep their minds and bodies strong. Our staff works closely together to improve the lives of those we serve.
