Our Community Care team provides intensive support services and medical care to people living with HIV/AIDS. Through our outreach in the community, we strive to help more people access their best course of care and enable their self-sufficiency.
The Dimock Center’s Medical Case Managers (MCMs) and Psychosocial Advocates help clients navigate our multidisciplinary clinic and help connect them to outside supports. Our Nurse Medical Managers provide medical care and are in close contact with the MCMs and Psychosocial Advocates. Clients can also access support groups, educational meetings, and referrals. Our staff works closely together to improve the lives of those we serve and empower Community Care patients to keep their minds and bodies strong.

Specialty Clinics and Services
Our Community Care patients benefit from services which include: medical case management, psychosocial advocacy, nurse medical management, primary care, HIV specialty care, OB/GYN, eye care, dental, nutrition, mental health, and housing services.
Integrated testing for Hepatitis C, Gonorrhea, Chlamydia, Syphilis, and HIV is provided by Community Care and available at various times and locations within the Roxbury community.
Our services are provided in English, Spanish, and Haitian Creole.
